Dog Training Submission Form

I understand that attendance of a dog training program is not without risk to myself, member of my family, guests who may attend, or my dog may be difficult to control and may be the cause of injury even when handled with the greatest amount of care. In consideration of and as inducement to the acceptance of my application for training, I hereby waive and release Landheim Training Center, its employees, officers, members, and agents from any and all liability of any nature, for injury or damage which I or my dog may suffer including specifically, but without limitation, any injury or damage resulting from the actions of any dog, and I expressly assume the risk of such damage or injury while attending any training sessions or any other function of Landheim Training Center Facility or while on the training grounds of the surround are thereto. I also agree to indemnify and to hold harmless Landheim Training Center, its employees, officers, members and agents from any and all claims, or claims by any member of my family or any other persons accompanying me to any training session or function of Landheim Training Center, or while on the grounds or the surrounding area thereto as a result of any actions by any dog, including my own. I shall personally assume all responsibilities and liabilities for any loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of my dog while participating in a Landheim Training Center Facility dog training program while on the training grounds or the surrounding area thereto. NO MONEY IS TO BE REFUNDED!!!!! It is the responsibility of each individual to attend scheduled classes, there are NO MAKEUP CLASSES. My dog and the above statement means any dog in my custody upon approaching the training grounds or adjacent areas.

Landheim Training Center agrees to exercise due and reasonable care to ensure the safety and welfare of all pets. That all pets are to be cared for properly according to instructions given by the owner. That housing quarters are kept clean and sanitary for the comfort of our guests. All pets are boarded, trained, groomed, or otherwise cared for without liability for the loss or damage due to (but not limited to) disease, running away, theft, fire or other unavoidable causes, due diligence and care having been exercised. Landheim Training Center is not to be held liable for loss or damage to personal property nor injury of other persons or pets caused by said Pet. The Pet will not be removed from Landheim property without written consent from Owner (unless health emergency). Kennel Cough is a virus that is highly contagious and not prevented completely by the vaccine. Only 3 of close to 20 strains of the virus are covered by the vaccine. Therefore, Landheim cannot be held responsible for you pet contracting the illness during their stay. In the unfortunate case that a Pet becomes seriously ill, the following protocol will take place: the Owner or emergency contact will be immediately notified. The designated Veterinarian will be notified. (However, Landheim reserves the right to seek veterinary care at our chosen facility.) After hours-North Central Emergency Veterinary Clinic will be notified. If required, the pet will be transported to said clinic exercising great care and safety. Landheim shall reserve the right to administer medication or give other advisable attention, within our discretion and judgment. The costs of such treatment and or supplies will be paid for by the pet Owner. In the case of boarding same family pets together: Landheim Training Center is not held liable for any injury or damage caused by either pet to one another. Payment is expected in full at the time of pick up of the Pet (due at drop off for In-Kennel Training). If the Pet is not picked up or called for within 15 days of its scheduled departure, Landheim Training Center reserves the right to obtain the Pet as its property to sell, place, or otherwise. The unpaid charges will then be turned over to a collection agency. By signing this contract (or providing an electronic signature) I agree that it is binding for this stay and any future stay. NO MONEY IS TO BE REFUNDED!!!!!

Here at Landheim our goal is to provide our customers with the best quality service possible. In order to achieve this and due to our high demand we have a few policies regarding our Classes. Due to high demand we require that all classes are paid in full at the time of registration to reserve your spot in class. No refunds are to be given per our training contract; however, we understand that extenuating circumstances can arise, and we may be able to offer money back as a courtesy to our customers. These will always be given at the discretion of the Operations Manager/Training Manager/Facility Owners. If a customer chooses to drop-out of a class for which he/she is registered or switch into a different class, we will only provide a refund/switch if we are able to fill the spot being vacated. Any customer wishing to drop-out of a class that has already begun will not receive a refund in any amount, no matter how many classes have been attended.

Here at Landheim our goal is to provide our customers with the best quality service possible. In order to achieve this and due to our high demand we have a few policies regarding our Private Lessons. Our private lessons are provided in 1-hour increments available Monday through Friday with the earliest lessons available with starting times from 9am through 4:30pm. We understand that sometimes it can be difficult to schedule a lesson during that time frame; therefore, we can occasionally provide lessons outside these hours for an additional fee. We will also require that all lessons are paid in full at the time of scheduling the appointment; cancellation will require 72-hour notice to receive refund or reschedule at no extra cost. These will be subject to availability of the arena and at the discretion of the trainers (customers with dogs in our In-Kennel Training program typically have lessons during these times and will be given priority). Due to safety concerns and scheduling availability, in-home lessons will incur additional fees and be provided solely at the trainer’s discretion. We will also require that at least one private lesson take place at our facility prior to an in-home appointment to determine necessity and address safety concerns.

Here at Landheim our goal is to provide our customers with the best quality service possible. In order to achieve this and due to our high demand we have a few policies regarding our In-Kennel Training Programs. Our In-Kennel Training programs require a deposit equal to ½ the total cost at time of reservation, the rest will be due upon dropping off your dog. To receive a refund or reschedule your pet’s in-kennel training stay at no extra cost, we will require notice 72 hours prior to the scheduled drop-off date. As we can only accommodate so many dogs in our program at a time, failure to drop-off your pet on your scheduled date or give adequate notice will result in forfeiture of your deposit. The lesson provided at the completion of the in-kennel training program is designed to help ensure successful maintenance of the training upon returning home with your dog. Due to high demand our lessons are scheduled tightly together and failure to arrive on time for your scheduled appointment may result in a decrease in your provided lesson time. We do not recommend owners return home with their dogs without receiving instruction on how to maintain the behavior's properly; therefore if your plans change and you are unable to pick up your dog at the arranged date/time we will try to accommodate to the best of our abilities but may result in additional charges.

In order to achieve this and due to our high demand we have a few policies regarding our Training programs. Once you have decided what training options you will be pursuing with your dog you may wish to come back and review these policies.

Due to the current health concerns if any individual in the household is not feeling well, we ask that you please remain home until everyone has completed the recommended quarantine or received a negative Covid test result. If this happens before your class is scheduled to start, notify Landheim immediately (219) 365-8897; we will attempt to switch your dog to another class at no extra charge, but only so long as we have sufficient time to fill your vacated spot. If this occurs while you are in class, we will send you the homework sheets for the week(s) you will miss so that you can return to class after the suggested quarantine cycle or upon receiving a negative test result. We also ask that you limit attendance to one (1) person attending class with your dog to maintain the required group limits unless impossible to do so. We also ask to refrain from bringing children to class for their safety and the safety of others. We thank you for your cooperation and understanding during this time.
